Make a referral

We have been developing and providing services for over 25 years and, as a result, now have over 100 beds for slow-stream rehabilitation and step-down independent living facilities. Although many of our referrals are made by healthcare professionals, we accept referrals from a range of stakeholders:

  • Clinical commissioning groups, commissioning support units and placement managers
  • NHS Trusts and NHS business partner organisations
  • Organisations requesting medical assessment in relation to legal accident claims
  • Law firms involved in legal solutions regarding accident and personal injury claims
  • Private patients and those with an insurance policy
  • Relatives, carers and friends

Once a referral is received, our admissions and discharge committee will triage the referral. If appropriate, a pre-admission assessment will be arranged and undertaken, the results of which will be shared with the referrer together with a proposed care package costing. If agreed by the funder, admission arrangements will be made and an admission date set.

It is our policy to regularly reassess patients to ensure that the placement, care plan and goals are appropriate and actively promote independence and support recovery.
