Referral process

Once a referral is received, this is triaged by our admission & discharge committee. If the referral is suitable, a pre-admission assessment will be conducted by members of the clinical team. As part of the assessment process, we encourage patients and families to visit the hospital prior to admission so that they can familiarise themselves with the environment and services provided.

Once the assessment is complete and the care package is agreed, our admission and discharge committee will ensure that everything is in-place to enable the admission to take place smoothly and at a time that suits all parties. Sometimes this will be at short notice but we will ensure all parties are involved and everything runs a smoothly as possible.

On the day of admission, you will be helped to settle in by your assigned keyworker(s). A number of tests and assessments will be arranged to ensure all your needs on admission are met. They are also designed to help the clinical team ensure that you get the right treatment.

Once admitted, patients and their families will be fully involved in formulating the care plan. Regular reviews of the care plan will take place to assess progress against agreed personal goals.
